| The Health Research & Educational Trust of NJ is seeking a Healthcare Navigator to conduct the day-to-day operations of a special pilot project in the surrounding areas of Paterson and Newark, NJ. |
This two-year, federally funded project will address the community-based factors that impact readmissions, as well as other forms of healthcare harm, by focusing on opportunities for change within the community. Individual will use the Health COACH model, provide outreach, screening for mental/behavioral health and social service needs, education, patient empowerment, care coordination and referrals.
Individual will establish partnerships and referral processes with area hospitals, FQHCs and other community-based providers, social service agencies, community organizations, and Veterans groups, for example. The Healthcare Navigator will conduct and/or participate in public education activities designed to raise awareness of services available; makes referrals or assists with accessing health/wellness support as needed. Helps to identify patients/consumers with unmet needs and barriers related to utilization of regular sources of care, unmet mental/behavioral health needs, self-management of chronic conditions and/or are at high risk of inappropriate/unnecessary hospital admissions, ED visits and potentially preventable readmissions. Will provide information and services in a manner that is culturally and linguistically appropriate to the needs of the population being served, ensuring information is provided in a fair, accurate and impartial manner, acting in the best interest of the consumer assisted.
Position requires participation and successful completion of both in-person and web-based training will be required for Health COACH, Mental Health First Aid, Veteran Service and NJ FamilyCare, as well as federal and state application counselor/assister certification. An Honorable discharge from any branch of the U.S. military along with a college degree or equivalent training/experience preferred. Applicants bilingual in English and Spanish (or other language prominent in region to be served) preferred. Experience conducting outreach and social service application assistance preferred. Ability to demonstrate creative problem-solving, organizational skills and peer education skills. Must have reliable source of transportation for daily travel to Paterson, Newark and surrounding areas.
